Why the title?

This title comes from a story my grandma used to tell us. I don't clearly remember the entire story, but it was about a man who didn't know where to defecate! (hence koja berinam!). My grandma had a number of these stories that had forbidden words and concepts. I can still see her and hear her voice, the way she would tilt her head and move her hands and would say koj berinam??
